network / asset documentation

hi guys,
i am researching for network inventory or machine profiles templates i.e. i want to make records for all the assets connected to the network. e.g some of the fields that i have idea are,

Machine: Desktop PC
Make:
Model:
Name: mmkhan
OS: Linux
IP: 192.18.2.11
MAC address:
Software installed:
Notes (Last activity):
etc
etc

I think such thing is necessary for network administrators and managers i.e. proper documentation of the network. I also got a template from internet http://www.diyplanner.com/node/419. This template is a good starting point for the beginners.

What do you guys use for this situation, what type of templates do you have? I am open to all type of suggestions.
