What information do you want to gain by doing this survey?

just trying to gather what kind of people would use the software , what features they would like for , what they would use it for etc.
Just a basic information on the audience , what the audience seeks etc .
The software itself is going to be similar to basecamp (thus since its a 4 man opensource project it wont have as many features to start with anyway)

I think you may be going about this oddly.
It would be easier if you had a solid idea that would serve as the core of the project: what YOU want it to do. This would answer the questions you mentioned about what the application would be used for and what features they would want added (since they would have a feel for what it would already do).
If you do want to continue with the direction you're going in, an idea would be to ask what people dislike about other available applications out there? What features they want but are not seeing fulfilled with basecamp (or another alternative)... try and tailor the project to those who are unhappy with what's available.
