Well as I am sure you know it is usually tea and coffee that are the culprits, but I did come across a weird one where the young lady was processing documents that were two sheets pinned together with a small staple. Get a few of those in the keyboard and strange things happen :D

An even better one in the same office was where the sysop had replaced three terminals one after the other. Turned out it was a desk fan that must have been about to give up the ghost and was spewing out some sort of EMF that was causing the problem.
