If this has been posted before, sorry.

It is possible for a virus to be backed up when your windows (ME & XP) system creates a restore point. Your virus scan can't clean the file, so you have to take an alternate action.

It is my understanding that this is possible in at least some cases. Case in point being the klez worm (if memory serves me correctly), where Symantec recommended that system restore be turned off before using the removal tool, and using restore points created prior to cleaning was discouraged for safe measure.
